The Social Security Administration (SSA) has issued a new SSA-89 form (version 12-2020) used to fulfill our Consent Based Social Verification (CBSV) service. The old version (02-2018) will still be accepted through September 2021, but we encourage you to begin using the new version, updated in our system, immediately.
